We have arranged for some doctors to come to Pittenweem to perform medicals on Monday 20th November 2023. These will be held at Pittenweem Bowling Club, at 25 Viewforth Place, Pittenweem, KY10 2PZ.
You must book an appointment time slot in advance. This can be done online at this link or by calling 01282 936900 (dial option 1).
Please remember to bring the following:
- Payment in cash or paid online in advance (£85 per person)
- Photo ID
- Any medication you are taking or documentation for that medication
- Glasses if required
- If you have any medical conditions, please bring any documentation
- Your registered GP’s Address (find this in advance to save time at your examination)
ML5 medical forms will be provided on the day, or can be emailed to you, or we have a few paper copies in the office.
